Please download the two waiver forms below, have them signed by a parent or legal guardian if you are under 18, and upload the completed forms using the submission form on this page. Players currently over 18 only need to sign the Participant Waiver

Both signed waivers must be received before you can take part in any All-Star Football Classic activities, including practices, events, or game day.
